New Hampshire Perinatal Substance Exposure Collaborative Family Care Planner

The NH Perinatal Substance Exposure Collaborative launched a pilot project featuring a comprehensive Family Care Planner designed to support expecting mothers throughout pregnancy and postpartum. Distributed by healthcare providers, the planner included medical forms, resource lists, and interactive worksheets to help parents prepare for their new baby.

Following the development and review of five mood boards, the team selected a nature-inspired line art direction. I translated this concept into a warm, supportive visual identity using soft, calming tones, delicate illustrations, and a friendly, approachable typeface. The accompanying website provides another example of the visual language in use, demonstrating how the identity extends beyond the planner into a cohesive digital experience.
